– Name, address and phone number of supervisor
– Salary
– Number of hours per week
– Dates of the training (Summer 2019: June 15 – September 25)

Step 3: Enroll into a CPT-Eligible Course… SUMMER COOPERATIVE PROGRAM FOR PHD
– Summer Cooperative Education Program

Step 4: Submit CPT Application… THROUGH IPORTAL THEN EMAIL DEPT. THEN EMAIL ISPO
– iportal.ucsd.edu
– Curricular Practical Training (CPT) I-20 Request
– I-94, Job Offer Letter, Summer Cooperative Education Agreement

Step 5: Pick up your CPT I-20… GO TO ISPO

Step 6: Submit your Post-Cooperative Education Report…
